A card that lowers the cost of health care for people on lower incomes: cheaper visits to the doctor or nurse you are enrolled with, free subsidised prescriptions, cheaper public transport in some regions, and help with emergency dental care and children's glasses. You can get one if you are 16 or older, a New Zealand citizen or permanent resident (or hold certain other residence-class visas), normally live in New Zealand, and your household income is under the limit for your family size. People on some benefits get one automatically; otherwise you apply to Work and Income with two proof-of-identity documents.
